Council, a Public Hearing will be given at the Howard F. Alden Memorial Auditorium, 85 George P. Hassett Drive,
City Hall, Medford, Massachusetts at 7:00 P.M.,

a petition by National Grid of North Andover, MA. and Verizon, New England.,
for permission to locate poles, wires, and fixtures including the necessary
sustaining and protecting fixtures, along and across the following public way:
Boston Ave.-National
Grid to install 1 SO Pole and relocate one 1 JO Pole. National Grid to install
325’ +/- of 6-5” primary duct, 105’ +/- of 2-5” primary duct, 10’ +/- of 4-5”
primary duct, (2) 3 phase primary risers, 1 new midspan pole 3310-50. Ngrid to
relocate Pole 3313 23” +/- North of existing location
Wherefore
it prays that after due notice and hearing as provided by law, it be granted a
location for and permission to excavate the public highways and to run and
maintain underground electric conduits together with such sustaining and
protecting fixtures as it may find necessary for the transmission of
electricity. Said underground conduits to be located substantially in
accordance with the plan filed herewith marked Boston Ave-Medford, Massachusetts No#
19054236 April 22, 2015

– Chief Engineer –Upon review of this petition, the Engineering Department
based on the Engineers review, that this work can proceed forthwith provided
the following requirements are met; No City Owned or private utilities or other
structures are adversely impacted. National Grid shall ensure that all sewer,
water and drain lines are marked prior to any excavation.
The
sidewalk and any other City owned property are restored. It is my understanding
that Tufts will be responsible for sidewalk and road restoration. Should the
responsibility change, National Grid must restore these areas to existing or
better condition. All temporary structures are removed and the public way
restored
As
built information associated with this work is accurately incorporated into the
as built drawings Tufts will submit to the Cit… National Grid will inform the
City of any newly installed duct bank that it determines in the future, will
NOT be used as the 105 ft. length of duct bank that extends from the building
service to Pole 3314
Before
beginning work the contractor shall notify Dig Safe and shall obtain applicable
permits from the Engineering Division. National Grid shall complete all
restoration in accordance with City of Medford
regulations and standards as well as for removing all debris related to its
work.

approval, the following amendment to the proposed Fiscal 2016 budget for the
City of Medford
for the period
July 1, 2015 to June 30, 2016: the addition of $251,325 for Library salary
and expenses as set forth in my letter to you of June 9, 2015.
I am also requesting a
technical correction to the OCD/Boards Budget and the Public Works
Non-Enterprise budgets. The Hormel
Commission budget was inadvertently included in the OCD/Board Budget. It should be reflected instead in the Public
Works Non-Enterprise budgets.
